*Health memberships plans with Access Healthcare Services, LLC are not health insurance and should not be purchased in place of medical insurance.

*Memberships are intended to provide members with access to health services provided by Access Healthcare Services, LLC. 

*Health membership plans are required for a minimum of 6 (six) months from the contracted initiation date and may be cancelled with no fewer than 30 (thirty) days notice. Cancellation will be processed on the last day of the paid service month.
